Grid collapse symptom of deep malaise

When does a man die? obviously when his heart stops beating. But does he really die at the moment of pronouncement of his death or were there many moments leading him to that final stage? Of course, when one dies instantaneously of a shock, say following an accident, such a death could be described as having occured at that particular moment. The failure of grids on Monday and Tuesday are not accidents caused by a human or technical momentary error. It's the culmination of a long series of wrong actions and inaction.
